Introduction

When suppressing the virus is not the same as curing the disease

Chronic hepatitis B remains a major global health burden despite the availability of an effective vaccine. Current standard therapies, namely nucleos(t)ide analogues and pegylated interferon, can suppress viral replication, but they rarely clear the hepatitis B surface antigen (HBsAg), often require long-term or lifelong treatment, and leave the persistent viral reservoir largely untouched; interferon is also limited by tolerability. High circulating HBsAg is thought to contribute to immune dysfunction, which is why reducing it is a central goal of functional-cure strategies. HBV-targeted antisense oligonucleotides are short, chemically modified nucleic acids designed to bind specific HBV RNA sequences and trigger their degradation or block their expression, lowering viral antigen production directly at its source. The approach is promising but not without limits: efficient and selective delivery to hepatocytes, durability of response after treatment, sequence coverage across HBV genotypes and integrated viral DNA, safety and tolerability of oligonucleotide chemistries, and the need for combination regimens that restore immune control all remain open questions.

Key features

Core capabilities of HBV-targeted ASO therapeutics

The properties that make antisense oligonucleotides a precise tool against hepatitis B.

Sequence-specific HBV RNA targeting
Designed to bind defined viral RNA sequences, enabling selective action on HBV transcripts.
HBsAg and viral antigen suppression
Reduces production of surface antigen and other viral products at the RNA level.
RNA degradation and expression inhibition
Mechanisms that cleave the target RNA or block its translation to shut down viral gene expression.
Chemically modified, nuclease-resistant chemistry
Backbone and sugar modifications improve stability, binding affinity, and duration of activity.
Hepatocyte-targeted delivery
Targeting approaches improve liver uptake and intracellular delivery while limiting off-target exposure.
Dosing, biomarkers, and combinations
Optimized regimens, biomarker-guided treatment, and pairing with other agents to pursue functional cure.
Challenges addressed

Why conventional HBV therapy falls short, and what stays hard

What HBV-targeted ASO therapeutics aim to address, and where the limits are.

01
Low rates of HBsAg loss
Standard antivirals suppress replication but seldom clear surface antigen. RNA-targeted agents aim to lower it directly
02
Long-term or lifelong treatment
Therapy is often continued indefinitely, with adherence and relapse concerns. Finite regimens are a central goal of functional-cure strategies
03
Persistent antigen burden and immune dysfunction
High viral antigen levels are associated with weakened immune control. Reducing antigen production may help restore it
04
Delivery, durability, and safety limits
Liver-selective uptake, durable response off treatment, genotype and integrated-DNA coverage, and tolerability remain hard
